ABSTRACT

Objective To explore the feasibility of preheating in 41 ℃ water bath for 30 minutes to correct the red blood cell parameters in the specimens containing high-titer cold agglutinins(CAs). Methods Two specimens containing high-titer CAs were selected during work,and the parameters of complete blood count at room temperature or after preheating in 37 ℃ or 41 ℃ water bath were compared.The smears were stained,and the distribution of red blood cells was observed with a microscope.Further,74 specimens without CAs were collected for complete blood count,and then the test results at room temperature and after preheating at 41 ℃ were compared. Results At room temperature,the specimens containing high-titer CAs showed significantly reduced red blood cell count(RBC)and hematocrit(HCT),abnormally increased mean corpuscular hemoglobin(MCH)and mean cell hemoglobin concentration(MCHC),abnormal percents of hemoglobin(HGB)and RBC,and aggregation of a large number of red blood cells.After being preheated at 37 ℃ for a certain time,the specimens demonstrated obviously improved parameters while still aggregation of a small number of red blood cells.After being preheated at 41 ℃ for 30 minutes,the specimens showed significantly increased RBC,normal HCT,MCH,and MCHC,and evenly distributed red blood cells.The 74 specimens without CAs showed the comparability was ≥80% between room temperature and preheating at 41 ℃ for 30 minutes or 60 minutes. Conclusion We can preheat the specimens containing high-titer CAs in a water bath at 41 ℃ to obtain accurate red blood cell parameters.

ABSTRACT

【Objective】 To study the effect of changes in red blood cell parameters and blood donation interval on ferritin level in platelet donors with ferritin deficiency, so as to provide experimental evidence for formulating targeted blood donor care strategies. 【Methods】 100 regular male platelet apheresis donors in April 2020 were selected to test the ferritin and red blood cell parameters. Blood donors were classified according to the annual blood donation numbers: 3~10, 11~15, and 16~24, and the difference of iron deficiency rate among groups was analyzed. They also were classified according to the detection value of ferritin: iron deficiency donors versus SF normal donors, and the changes of Hb, MCV, MCH and MCHC values of them were compared. The iron deficiency donors were stratified by donation time: January to March 2019, April to June 2019, July to September 2019, October to December 2019 and January to April 2020, so as to compare the changes of Hb, MCV and MCH values among each group. The iron deficiency donors, continued the donation afterwards, were divided into interval-extended donors (minimum donation interval of 30 days) and normal ones (minimum donation interval of 14 days), and blood donation from May to December 2020 was intervened and observed. In December 2020, serum ferritin was tested again for iron deficiency donors to analyze the changes in SF between two groups. 【Results】 The incidence of iron deficiency in three groups with different donation frequency was 7.5%, 22.5% and 40.0%, respectively(P0.05), and original 13.89±4.66 vs current 35.92±5.20 in the extended ones(P<0.05). 【Conclusion】 The incidence of iron deficiency in apheresis platelet donors increased as blood donation frequency elevated. The Hb, MCV, MCH and MCHC value of iron deficiency donors decreased while the dispersion of pre-collection Hb, MCV and MCH decreased. MCV and MCH are superior to Hb in early-reflection of metabolism status of red blood cell. It is recommended that iron deficiency donors with Hb less than 130 g/L should reduce the frequency of donation. Extending the donation interval can increase the SF level of iron deficiency donors.Dynamic monitoring of ferritin and related red blood cell parameters should be carried out by blood banks to customize the donation interval.

ABSTRACT

Objective To investigate the changes of erythrocyte parameters and the value of differential diagnosis in pregnant women with β-mediterranean anemia.Methods A total of 300 pregnancy women from July 2014 to December 2015 in Center Hospital of Longgang were recruited in this study,100 pregnant women with β-mediterranean anemia in β-mediterranean anemia pregnancy group,100 healthy pregnant women in normal pregnancy group,100 pregnant women with iron deficiency anemia in iron deficiency anemia pregnancy group.Mean red cell volume(MCV),mean erythrocyte hemoglobin(MCH),reticulocyte percentage(Ret%) were detected and compared in the three groups.Results Compared with the normal pregnancy group and iron deficiency anemia pregnancy group,the MCV,MCH significantly reduced,and Ret% significantly rised in the β-mediterranean anemia pregnant group,the differences were significant(P<0.05).The best cut-off value of Ret% was 1.7% in differential diagnosis of β-mediterranean anemia pregnancy and iron deficiency anemia pregnancy,the sensitivity was 63.00%,the specificity was 74.00%,the area under of receiver operating characteristic curve was 0.841.The sensitivity of joint detection including MCV,MCH and Ret% in differential diagnosis of β-mediterranean anemia pregnancy and iron deficiency anemia pregnancy was 84.00%,the specificity was 90.00%.Conclusion MCV,MCH and Ret% in pregnancy women with β-mediterranean anemia changes significant compared with normal pregnancy group and iron deficiency anemia pregnancy group,the joint detection including MCV,MCH and Ret% could significantly improve the differential diagnosis of β-mediterranean anemia and iron deficiency anemia in pregnancy women.

ABSTRACT

Objective To explore the reason of serious interference red blood cell(RBC)parameters in blood routine test and correct method.Methods 30 cases of patients whose results of blood routine RBC parameters obviously abnormal ,According to the interference factors were divided into four groups:cold agglutination (7 cases),severe hemolysis(13 cases),high triglycerides (8 cases),high white blood cell(WBC)(2 例).Take appropriate corrective action for processing respectively.The difference of red blood cell parameters were compared with t test.Results To analysis of interference factors and take appropriate corrective meas-ures,RBC parameters could be more close to the real results.Conclusion We should pay attention to the changes of the RBC pa-rameters,analysis of interference factors,and to take appropriate measures to correct,in order to avoid clinical misdiagnosis and mi-stherapy.
